Wegovy Maintenance Dose: Do You Lose More Weight on a Higher Dose?

Wegovy Maintenance Dose: Do You Lose More Weight on a Higher Dose?

Wegovy maintenance is 2.4 mg or 1.7 mg, with 7.2 mg now an option. Trial weight loss by dose (15.6% vs 18.7% on 7.2 mg), side effects and cost per dose.

Yes, on average you lose more weight on a higher dose of Wegovy, but the extra loss is moderate and comes with more side effects. The label sets the standard maintenance dose at 2.4 mg a week, with 1.7 mg as an alternative for people who do not tolerate 2.4 mg, both reached after a 16-week build-up. In the STEP 1 trial, 2.4 mg produced an average loss of 14.9% of body weight over 68 weeks, against 2.4% on placebo. Since March 2026 there is also a higher maintenance option, Wegovy HD 7.2 mg, for adults with obesity who have taken 2.4 mg for at least four weeks: in the STEP UP trial it produced 18.7% weight loss against 15.6% on 2.4 mg, about three percentage points more, and twice as many people lost a quarter of their body weight. It also brought more stomach side effects and a new one, a burning or tingling skin sensation, in about 22% of people. No trial has compared 1.7 mg with 2.4 mg directly in people without diabetes, so how much less weight 1.7 mg produces has to be estimated. This guide explains each maintenance dose, the trial results, the side-effect trade-off and the cost.

Wegovy Maintenance Dose: The Short Answer

    1. 1Standard maintenance dose: 2.4 mg once a week (recommended), or 1.7 mg if 2.4 mg is not tolerated.
    2. 2Higher option: Wegovy HD 7.2 mg, approved March 2026, after at least 4 weeks on 2.4 mg.
    3. 32.4 mg: about 15% average weight loss at 68 weeks (STEP 1).
    4. 47.2 mg vs 2.4 mg: 18.7% vs 15.6% at 72 weeks (STEP UP); 31% vs 15% lost at least a quarter of their weight.
    5. 51.7 mg vs 2.4 mg: no direct trial in people without diabetes; dose-finding data suggest somewhat less loss on lower doses.
    6. 6Higher doses mean more side effects: gut symptoms in 71% on 7.2 mg vs 61% on 2.4 mg, and skin-sensation changes in about 22% vs 6%.

What Is the Wegovy Maintenance Dose?

Wegovy is started low and increased every four weeks: 0.25 mg in weeks 1 to 4, 0.5 mg in weeks 5 to 8, 1 mg in weeks 9 to 12, and 1.7 mg in weeks 13 to 16. From week 17 the label says the maintenance dose in adults is either 2.4 mg, which is recommended, or 1.7 mg once weekly. The slow build-up is there to reduce nausea and other gut side effects, as explained in the Wegovy dosing schedule. Since March 19, 2026, adults with obesity who have tolerated 2.4 mg for at least four weeks, and for whom more weight loss is clinically indicated, can move up to Wegovy HD 7.2 mg.

Do You Lose More Weight on 2.4 mg Than on 1.7 mg?

Probably somewhat more, but the exact difference is not known, because no trial in people without diabetes has randomized participants to 1.7 mg versus 2.4 mg as maintenance doses. The clearest evidence of a dose effect comes from two sources. The phase 2 dose-finding trial of daily semaglutide found weight loss rising with dose over 52 weeks, from 6.0% at the lowest dose to 13.8% at the highest, against 2.3% on placebo. And in STEP 2, in people with type 2 diabetes, 2.4 mg produced 9.6% weight loss against 7.0% on 1.0 mg and 3.4% on placebo. Together these suggest that lower doses produce less weight loss on average, but many people on 1.7 mg still lose a substantial amount, and individual responses vary widely. Whether staying on a lower dose makes sense for you is discussed in can you stay on a low dose of Wegovy.

How Much More Weight Do You Lose on 7.2 mg?

The STEP UP trial, published in 2025, enrolled 1,407 adults with a BMI of 30 or more and no diabetes, with an average weight of 113 kg. Over 72 weeks, counting everyone regardless of whether they stopped treatment, weight loss averaged 18.7% on 7.2 mg, 15.6% on 2.4 mg and 3.9% on placebo. Among people who kept taking the drug, the figures were 20.7%, 17.5% and 2.4%. The biggest difference was at the top end: 31.2% of people on 7.2 mg lost at least a quarter of their body weight, against 15.3% on 2.4 mg. At the bottom end there was almost no difference: about 90% in both groups lost at least 5%. In a separate trial in people with type 2 diabetes, 7.2 mg produced 13.2% weight loss.

Do Side Effects Get Worse at Higher Doses?

Yes. Side effects of semaglutide rise with dose, which is why the label allows 1.7 mg as maintenance and why Wegovy HD is reserved for people who already tolerate 2.4 mg. The newer 7.2 mg dose adds a side effect that was rare at lower doses: dysesthesia, an altered skin sensation such as burning, tingling or sensitivity to touch.

Interestingly, discontinuation rates for side effects were similar on 2.4 mg and 7.2 mg in STEP UP, though more people on 7.2 mg needed a dose reduction and fewer reached the full target dose (75% against 89%). Is It OK to Stay on a Lower Dose?

Yes, within the label. The FDA-approved maintenance dose is 2.4 mg or 1.7 mg, so staying on 1.7 mg is an approved option, not a workaround. For someone who has good results and few side effects on 1.7 mg, the extra loss from 2.4 mg may not be worth more nausea. For someone whose weight loss has stalled on 2.4 mg, moving to 7.2 mg is the newer option, but only after at least four weeks on 2.4 mg; you cannot go straight from 1.7 mg to 7.2 mg. A plateau has many possible causes, discussed in why am I not losing weight on Wegovy. Some insurance plans' renewal criteria refer to a stable maintenance dose, which includes 1.7 mg, as explained for one insurer in does Aetna cover Wegovy.

    What About the Wegovy Pill?

The Wegovy pill, approved in December 2025, is taken daily and built up monthly to a 25 mg maintenance dose. In the OASIS 4 trial it produced 13.6% weight loss over 64 weeks against 2.2% on placebo, or 16.6% among people who kept taking it, similar to the 2.4 mg injection. The two forms are compared in Wegovy pill vs injection.

What Happens If You Stop at the Maintenance Dose?

In the STEP 4 trial, everyone took 2.4 mg for 20 weeks and lost an average of 10.6%. Those who continued lost a further 7.9% over the next 48 weeks, while those switched to placebo regained 6.9%. Stopping usually means regaining much of the weight, whatever the maintenance dose, as discussed in what happens when you stop taking Wegovy.

When to Talk to Your Doctor

    1. 1If side effects on 2.4 mg are hard to tolerate, to discuss staying on 1.7 mg
    2. 2If your weight loss has stalled on 2.4 mg, to discuss whether 7.2 mg is appropriate
    3. 3If you develop burning, tingling or painful skin, particularly after moving to 7.2 mg
    4. 4If you have missed two or more doses in a row, before restarting
    5. 5If you have severe abdominal pain, persistent vomiting or signs of dehydration

    The Bottom Line

Higher doses of Wegovy produce more weight loss on average: about 15% on 2.4 mg and about 19% on 7.2 mg in the trials, with the biggest gains in the share of people losing a quarter of their weight. Lower doses probably produce somewhat less, though 1.7 mg has not been tested head to head. Each step up also raises the chance of side effects, especially gut symptoms and, at 7.2 mg, skin-sensation changes. The best maintenance dose is the one that gives you meaningful, sustainable weight loss with side effects you can live with, decided with your prescriber.

    FAQ

Yes, 1.7 mg is an FDA-approved maintenance dose alongside the recommended 2.4 mg. Many people stay on it if 2.4 mg causes too many side effects.

Not any more. Since March 2026, Wegovy HD 7.2 mg is approved for adults with obesity who have tolerated 2.4 mg for at least four weeks.

No. Wegovy HD is for people who have taken 2.4 mg for at least four weeks.

In STEP UP, about three percentage points more on average: 18.7% versus 15.6% at 72 weeks. Twice as many people lost at least 25% of their body weight on 7.2 mg.

It is dysesthesia, an altered skin sensation reported by about 22% of people on 7.2 mg against 6% on 2.4 mg in STEP UP. Tell your prescriber if it happens.

    Through NovoCare self-pay, 7.2 mg costs $399 a month against $349 for 1 mg to 2.4 mg.
