Does Blue Cross Blue Shield Cover Wegovy? 2026 Changes by State, Prior Authorization and What to Do If the Answer Is No
BCBS Wegovy coverage depends on your Blue plan: Massachusetts and Vermont dropped it in 2026, Michigan in 2025, FEP Blue covers with PA. How to check.
Written by Diana Gangan. Updated October 29, 2026.
Whether Blue Cross Blue Shield covers Wegovy depends on which Blue plan you have, and in 2026 the answer has split sharply by state. Blue Cross Blue Shield is not one insurer but a federation of independent companies, each of which sets its own drug list, and several of them have dropped weight-loss drugs altogether: Blue Cross Blue Shield of Massachusetts stopped covering Wegovy for obesity from January 1, 2026, with no exceptions, Blue Cross Vermont did the same for most plans, and Blue Cross Blue Shield of Michigan ended coverage for its large-group commercial plans at the start of 2025. Others still cover Wegovy with prior authorisation, including the Federal Employee Program, which lists it on all three of its options. Almost every Blue plan still covers semaglutide when it is prescribed for type 2 diabetes, which is why the same drug can be covered as Ozempic and refused as Wegovy. This guide explains how to find out what your plan does, what prior authorisation usually requires, the state changes confirmed so far, and what to do if the answer is no.
Does Blue Cross Blue Shield Cover Wegovy? The Short Answer
- 1It depends on your Blue company and your employer's plan. There is no single BCBS policy.
- 2Dropped for weight loss in 2025–2026: BCBS Massachusetts (Jan 1, 2026, no exceptions, including heart disease), Blue Cross Vermont (Jan 1, 2026, most plans), BCBS Michigan large-group commercial (phased out Aug 2024 to Jan 2025).
- 3Still covered with prior authorisation on many other Blue plans, including the Federal Employee Program (FEP Blue), where Wegovy is on all three options.
- 4Typical criteria: BMI of 30 or more, or 27 or more with a weight-related condition, plus documented diet and exercise efforts; renewal often requires at least 5% weight loss.
- 5Type 2 diabetes drugs (Ozempic, Mounjaro, Rybelsus/Ozempic tablets) remain covered with prior authorisation on almost every Blue plan.
- 6If you are not covered: NovoCare self-pay is $349 a month for the injection and from $149 for the pill; Medicare's GLP-1 Bridge charges $50 a month from July 2026.
Why 'Blue Cross Blue Shield' Has No Single Answer
The Blue Cross Blue Shield Association licenses the brand to dozens of independent, locally operated companies: Anthem in many states, Highmark in Pennsylvania and New York, Florida Blue, CareFirst in Maryland and Washington DC, Horizon in New Jersey, and separate Blue companies in most other states, plus the Federal Employee Program run jointly for federal workers. Each writes its own formularies and medical policies. On top of that, most people with employer coverage are in self-insured plans, where the employer, not the insurer, decides whether weight-loss drugs are included and the Blue company only administers the benefit. Two colleagues with the same Blue card can therefore get different answers if one works for a company that bought the weight-loss rider and the other does not. The general rules that apply across insurers are in does insurance cover Wegovy.
Blue Plans That Changed Coverage in 2025–2026
Confirmed Blue Cross Blue Shield changes to Wegovy coverage for weight loss
Blue Cross Blue Shield of Massachusetts
Wegovy, Zepbound and Saxenda excluded for obesity and for other uses such as heart-disease prevention; no exceptions or appeals; employer rider is the only route
Plan renewals from Jan 1, 2026
GLP-1s for type 2 diabetes (Ozempic, Mounjaro, Rybelsus, Trulicity) with prior authorisation
Blue Cross Vermont
GLP-1 drugs approved only for weight loss no longer covered on Qualified Health Plans, Blue Edge Business and most employer plans
Jan 1, 2026
Diabetes GLP-1s with prior authorisation; Wegovy may be covered to reduce cardiovascular events under SELECT-based criteria
Blue Cross Blue Shield of Michigan
Weight-loss GLP-1s (Wegovy, Zepbound, Saxenda) phased out for large-group commercial plans
Phase-out from Aug 1, 2024; ended by Jan 1, 2025
GLP-1s for type 2 diabetes
Federal Employee Program (FEP Blue)
Wegovy covered with weight-management prior authorisation on Standard, Basic and Focus; tier and cost differ by option
2026 plan year
Zepbound preferred on FEP Blue Focus; formulary exception on Standard and Basic
These are the changes confirmed in insurer documents or reported in the press. Many other Blue companies have not announced blanket exclusions but restrict coverage through prior authorisation, step therapy (trying another drug first) or employer choice. Coverage can change at each plan renewal, usually January 1, so check every year.
How to Check Whether Your Blue Plan Covers Wegovy
- 1Find your company: the name on your card (for example, Anthem Blue Cross, Florida Blue, Highmark, CareFirst) tells you which Blue company's rules apply.
- 2Search your plan's formulary (drug list) on the member website or app for 'Wegovy' and note the tier and any 'PA' (prior authorisation), 'ST' (step therapy) or 'QL' (quantity limit) codes.
- 3Check whether weight-loss drugs are excluded as a benefit: some plans list the drug but exclude the 'anti-obesity medication' category in the benefit booklet.
- 4Call member services and ask three questions: is Wegovy covered for weight management, what are the prior authorisation criteria, and is my plan self-insured.
- 5If you work for an employer, ask HR whether the plan includes the weight-loss medication benefit or rider.
- 6Ask your prescriber's office: they submit prior authorisations every day and usually know the local Blue plan's criteria.
What Prior Authorisation Usually Requires
Where Blue plans cover Wegovy for weight management, prior authorisation criteria are broadly similar, though the details vary by company and plan. Expect most of the following.
Typical Blue plan prior authorisation criteria for Wegovy (weight management)
Age
18 or older (some plans allow 12+, matching the label)
Adolescent use usually needs a specialist
BMI
30 or more, or 27 or more with a weight-related condition
High blood pressure, high cholesterol, type 2 diabetes, sleep apnea, heart disease
Lifestyle programme
Documented diet, exercise or behavioural programme, often 3 to 6 months
Notes from your clinician usually count
Step therapy
Some plans require a trial of an older drug first (for example phentermine-topiramate or naltrexone-bupropion)
Ask whether exceptions apply if you cannot take them
Initial approval
Often 3 to 6 months
Covers the escalation phase
Renewal
Commonly at least 5% weight loss from baseline
FEP Blue requires documented loss at the 6-month renewal
Quantity
Four pens per 28 days; 30-day supplies
Mail order may be required
The dose ladder that the authorisation has to cover is described in the Wegovy dosing schedule. Since the 2025 label update, 1.7 mg is listed as a maintenance dose alongside 2.4 mg, which can matter for renewals if 2.4 mg was not tolerated, as explained in can you stay on a low dose of Wegovy.
Wegovy for Heart Disease: A Separate Question
Wegovy has two kinds of indication: chronic weight management, and reducing the risk of heart attack, stroke and cardiovascular death in adults with established heart disease and overweight or obesity, based on the SELECT trial. Some plans that exclude weight-loss drugs still cover Wegovy for the cardiovascular indication, because it is a treatment for heart disease rather than for weight. Blue Cross Vermont's 2026 policy, for example, keeps a route for Wegovy prescribed to prevent cardiovascular events. Others do not: Blue Cross Blue Shield of Massachusetts states that its exclusion applies to heart-disease prevention as well. If you have had a heart attack, stroke or peripheral artery disease, ask your prescriber to submit the prior authorisation under the cardiovascular indication. The Wegovy pill carries the same indications as the injection, as compared in Wegovy pill vs injection.
What to Do If Your Blue Plan Says No
- 1Appeal, if your plan allows it: ask for the denial in writing, have your prescriber submit a letter of medical necessity, and request a peer-to-peer review. Note that benefit exclusions, such as BCBS Massachusetts', cannot be appealed.
- 2Ask about the cardiovascular indication if you have established heart disease.
- 3Ask HR whether your employer can add the weight-loss rider at the next renewal.
- 4Check the alternatives your plan does cover: Zepbound, Saxenda or older oral drugs may be on the formulary even if Wegovy is not; the comparison is in Zepbound vs Wegovy.
- 5Use self-pay: NovoCare sells the Wegovy injection for $349 a month and the pill from $149; the Novo list price for Wegovy falls to $675 a month on January 1, 2027. Details and coupons are in Wegovy cost and savings.
- 6If you are on Medicare: the GLP-1 Bridge, running from July 1, 2026 to December 31, 2027, covers Wegovy for weight loss at $50 a month for eligible people with a BMI of 35 or more, or 27 or more with certain conditions.
Wegovy vs Ozempic Coverage on Blue Plans
The same molecule, semaglutide, is sold as Ozempic for type 2 diabetes and as Wegovy for weight management. Almost every Blue plan covers Ozempic for diabetes with prior authorisation, including the plans that have excluded Wegovy. That has led some people to ask for Ozempic instead. Blue plans now routinely check for a diabetes diagnosis, and prescribing Ozempic off-label for weight loss to obtain coverage is the kind of claim insurers audit and deny. If you have type 2 diabetes, Ozempic or Mounjaro is the right route anyway, and the dose comparison is in how much weight you can lose on Wegovy.
Why Blue Plans Are Dropping Weight-Loss Drugs
The reason insurers give is cost. Weight-management GLP-1s are taken long term, often indefinitely, at list prices above $1,000 a month until recently, and millions of members qualify. Blue Cross Blue Shield of Michigan cited the cost to employers and members when it announced its exclusion. Two developments may shift the calculation: Novo Nordisk's list-price cut for Wegovy to $675 from January 2027, and growing evidence that stopping the drug leads to regaining most of the weight, which makes intermittent coverage less useful for everyone, as discussed in what happens when you stop taking Wegovy.
When to Talk to Your Doctor
- 1Before your plan renews, if you are on Wegovy and your Blue company has announced a change: plan the transition rather than stopping abruptly
- 2If you have heart disease: ask whether a prior authorisation under the cardiovascular indication is possible
- 3If you have type 2 diabetes: ask whether a diabetes GLP-1 is more appropriate and covered
- 4If you are paying out of pocket and considering compounded semaglutide: discuss the risks, since compounding of semaglutide has been restricted since 2025
- 5If a denial arrives: ask your prescriber's office to help with the appeal and the letter of medical necessity
The Bottom Line
Blue Cross Blue Shield covers Wegovy on some plans and not others, because each Blue company and many employers make their own decisions. Massachusetts and Vermont dropped weight-loss coverage for most members on January 1, 2026, Michigan's large-group plans did so a year earlier, and the Federal Employee Program still covers Wegovy with prior authorisation. Check your formulary, ask member services the three questions above, consider the heart-disease route if it applies, and know the self-pay and Medicare Bridge prices if the answer is no. What Wegovy does and what to expect once you start is covered in what Wegovy does.
FAQ
It depends on your Blue company and employer. BCBS Massachusetts and Blue Cross Vermont stopped covering weight-loss GLP-1s for most plans on January 1, 2026, and BCBS Michigan large-group plans ended coverage in 2025. Other Blue plans, including FEP Blue, cover Wegovy with prior authorisation.
Plans that cover Wegovy for weight management typically require a BMI of 30 or more, or 27 or more with a weight-related condition such as high blood pressure, high cholesterol, type 2 diabetes or sleep apnea, plus documented lifestyle efforts.
Yes, with a weight-management prior authorisation on FEP Blue Standard, Basic and Focus in 2026. The cost depends on the option and tier, and renewal requires documented weight loss.
Some Blue plans do even when they exclude weight-loss drugs; Blue Cross Vermont keeps a route under SELECT-based criteria. BCBS Massachusetts excludes Wegovy for heart-disease prevention as well. Ask for the prior authorisation under the cardiovascular indication.
Ozempic is covered for type 2 diabetes, which almost all Blue plans treat as essential. Wegovy is covered under a weight-management benefit that many plans and employers have excluded, even though both contain semaglutide.
Through NovoCare, $349 a month for the injection and from $149 for the lower-strength pills. The Wegovy list price falls to $675 a month from January 1, 2027. Medicare enrolees may qualify for the $50 GLP-1 Bridge.
