How Long Does Wegovy Take to Work? A Week-by-Week Timeline
How long Wegovy takes to work: appetite changes in days to weeks, slow first month on 0.25 mg, steady loss from month 3, and a plateau around a year.
Written by Diana Gangan. Updated October 22, 2026.
Wegovy starts working within days, but it takes months to see its full effect. Semaglutide peaks in the blood one to three days after each injection, and many people notice less hunger in the first week or two. Weight loss is usually slow during the first month on the 0.25 mg starter dose, speeds up once you reach the higher doses, and keeps going for most of the first year. In the main trial, weight was still falling until around week 60.
Here's a realistic week-by-week timeline for the injection and the pill, and what can slow things down.
Wegovy Timeline at a Glance
What to expect after starting the Wegovy injection
Days 1–3
0.25 mg
Peak drug level; mild nausea or fullness for some, nothing for others
Weeks 1–4
0.25 mg
Smaller appetite for many; little change on the scale
Weeks 5–16
0.5 → 1 → 1.7 mg
Appetite clearly lower; steady weight loss begins; side effects can flare after each step
Week 17 onward
2.4 mg maintenance
Most consistent effect; weight keeps falling for months
Around 6–12 months
Maintenance
Weight loss slows and levels off for most people
Why the First Month Feels Slow
The starter dose, 0.25 mg, is there to let your stomach adjust, not to cause big weight loss. Semaglutide also takes four to five weeks of weekly doses to reach a steady level in the blood, and that clock restarts with each dose increase. Most of the meaningful weight loss happens once people are on 1 mg or more, from roughly the third month.
How Much and How Fast
In STEP 1, adults on Wegovy 2.4 mg lost about 15% of their body weight over 68 weeks on average, with weight still dropping until about week 60. About a third lost 20% or more; some lost much less. Wegovy HD, 7.2 mg, produced about 21% over 72 weeks for people who stepped up beyond 2.4 mg. See Zepbound vs Wegovy for how that compares.
The Wegovy Pill Timeline
The pill follows a similar arc on a different ladder: 1.5 mg daily for 30 days, then 4 mg, 9 mg and 25 mg from day 91. Average weight loss was about 13.6% over 64 weeks. Because absorption depends on taking it correctly every morning, inconsistent use slows results more than with the weekly injection. See Wegovy pill vs injection.
What Can Slow Wegovy Down
- 1Staying on a low dose because of side effects. That's sometimes the right call, but the effect is smaller.
- 2Missed doses. If your next injection is less than two days away, the label says to skip it, which costs you a week.
- 3Taking the pill with coffee, food or a full glass of water.
- 4Liquid calories and frequent snacking that work around a smaller appetite.
- 5A heat-damaged pen. See our storage guide.

When to Talk to Your Prescriber
Don't judge Wegovy on the first month. If you've been on a full maintenance dose for about three months and lost less than 5% of your starting weight, that's a reasonable point to review the dose, your routine, or whether another medicine would suit you better. Seek help sooner for severe stomach pain, vomiting you can't stop, or signs of dehydration. See side effects.
Bottom Line
Wegovy works on appetite within days to weeks, but the scale moves slowly at first and most weight loss comes after you reach the higher doses, from about month three. Expect steady loss for most of the first year. Judge it on the maintenance dose, not the starter dose. The full dose ladder is in our Wegovy dosing schedule.
FAQ
It peaks in the blood one to three days after a shot, and many people notice less appetite in the first week or two. Visible weight loss usually builds from the second or third month.
In the first month on 0.25 mg, often only a few pounds. Loss speeds up on higher doses.
16 weeks for the injection (2.4 mg from week 17) and 90 days for the pill (25 mg from day 91).
For most people, weight loss slows toward the end of the first year. In STEP 1, it leveled off around week 60.
Common reasons are still being on a low dose, missed doses, taking the pill incorrectly, or eating patterns. Review with your prescriber after about three months at full dose.
